Home Health FAQs

The reality of health care is that hospital stays are shorter.  CharterCARE Home Health Services is available to assist you during your recovery period. Our main goal is to get our patients back on their feet, in the safety and comfort of their own home, under the supervision of your entire care team, including physicians, visiting nurses, therapists, CNA’s and medical social workers.

Who is eligible for HomeCare?

HomeCare is available for anyone over age 18. Our HomeCare is available to anyone throughout Rhode Island and bordering parts of Massachusetts. You must be under the care of a physician and in need of skilled nursing or rehabilitation services. Our home care patients may have been recently discharged from a hospital or nursing home or they may reside in an assited living facility.

Who provides the care?

Our home care team is comprised of registered and licensed nurses, licensed physical therapists, occupational therapists, speech therapists, CNAs, medical social workers or a palliative care nurse practioner.

How can CharterCARE Home Health Services restore your independence?

You will remain in the comfort of your own home surrounded by your favorite things. We can assist in restoring your daily functions such as, cooking your favorite meal, resuming your hobbies or maintaining your home.

Is home care covered by insurance?

Most insurance companies cover the cost, as long as care is provided under the supervision of a registered nurse or a physical or speech therapist as part of a treatment plan prescribed by a physician.

Are HomeCare services available 24/7?

Absolutely. CharterCARE Home Health Services are available 365 days a year and we have a clinican on-call during our off office hours. Holidays are no exception.

How is CharterCARE Home Health Services different?

We are part of a larger health care network. You will have the ability to move from our acute care setting, to sub-acute, to our elder friendly nursing facility or receive any of our outpatient services.

Do you have to be a patient in a CharterCARE facility to receive CharterCARE Home Health Services?

No. Our home care agency accepts referrals from other area hospitals, outpatient clinics, ambulatory surgery centers, emergency departments, doctors’ offices, skilled nursing facilities, patients, and families.
