Our medical social service providers are available for in-home visits to assess patient psychosocial needs whenever a skilled discipline is involved.
Social workers provide:
- Psychosocial evaluation of home and family dynamics.
- Assistance with long-term care options (e.g., assisted living or nursing facilities).
- Education and assistance regarding health, legal, and/or financial matters.
- Short-term counseling services.
- Referral to and coordination with community resources.
